Good Faith Estimate
Good Faith Estimate
AlderMind Wellness, PLLC
EFFECTIVE DATE: September 21, 2026
Understanding the expected cost of your care is an important part of making informed healthcare decisions. Under the No Surprises Act, patients who are uninsured or choose not to use their insurance have the right to receive a written Good Faith Estimate for non-emergency healthcare services.
Your Good Faith Estimate outlines the anticipated charges for your scheduled care, including other reasonably expected items or services associated with that care. You may also request an estimate before scheduling an appointment.
The following timeframes apply:
- Appointments scheduled 3–9 business days in advance: Your estimate will be provided within one business day after scheduling.
- Appointments scheduled 10 or more business days in advance: Your estimate will be provided within three business days after scheduling.
- Estimates requested before scheduling: Your estimate will be provided within three business days of your request.
A Good Faith Estimate is based on the information available when it is prepared. Actual charges may vary if your healthcare needs change or additional services become necessary.
A Good Faith Estimate is not a contract. It does not require you to obtain the items or services included in the estimate from AlderMind Wellness or from any provider or facility identified in the estimate.
If a provider’s bill is $400 or more above that provider’s Good Faith Estimate, you may be eligible to challenge the charges through the federal patient-provider dispute resolution process. Please retain a copy of your estimate and any related bills.
To request an estimate or ask questions about your anticipated charges, contact AlderMind Wellness at 360-226-8886. Additional information about your rights is available at cms.gov/nosurprises/consumers.
